Disciplinary-Proceedings: $25,000 aggregate limit of liability.
Lost Earnings: $1,000 per day, with an aggregate limit of liability of $35,000.
Medical Payments:$25,000 per occurrence with a $100,000 aggregate limit.
Emergency Aid Expenses: $10,000 aggregate limit.

If you employ any staff, it’s essential to include them on the application. Indicate yourself as the owner and list any W2 employees under question #4, while 1099 contractors should be noted under question #11.
