Limits of Liability: $1,000,000/$3,000,000.

Supplemental Coverages / Limits

Information Privacy Wrongful Acts: $25,000 aggregate limit of liability.
Disciplinary-Proceedings: $25,000 aggregate limit of liability.
Lost Earnings: $1,000 per day, with an aggregate limit of liability of $35,000.
Medical Payments:$25,000 per occurrence with a $100,000 aggregate limit.
Emergency Aid Expenses: $10,000 aggregate limit.
Damage to Property of Patients: $10,000 aggregate limit.

A listing of owners and/or partners indicating the percentage of the business owned by each
A detailed description of the services you offer clients. The letter of services can also include the population you serve; the diagnoses you treat, and the treatment modalities you utilize. If you have a brochure or website, please include that as well.
If you employ any staff, it’s essential to include them on the application. Indicate yourself as the owner and list any W2 employees under question #4, while 1099 contractors should be noted under question #11.
